Why registration English catches out strong candidates

The requirement for AHPRA / the Nursing and Midwifery Board of Australia is IELTS Academic 7.0 in each of the four skills. — and it is a substantially harder bar than the same test set for a university place. Clinicians who use English every working day routinely miss it, for reasons that have very little to do with their clinical English.

• It is per skill, so your lowest score is your result. An overall figure that comfortably clears the bar counts for nothing if one skill sits below it. That single rule is the most common reason a registration application stalls, and it means your preparation should be aimed almost entirely at your weakest skill rather than spread evenly.
• Writing is usually the one that fails. Across registration routes it is the most commonly re-sat skill, and the reason is structural rather than linguistic: IELTS Academic Writing asks you to describe a chart and argue an abstract position to an academic marking scheme. Neither resembles clinical documentation, so years of writing excellent notes and discharge summaries builds very little of what Task 1 and Task 2 are marked on.
• Check whether scores can be combined across sittings before you re-sit. OET at grade B in each section is also accepted; results can be combined across two sittings under set conditions. Where combining is permitted the conditions are strict — typically both sittings within a set window and no skill below a floor — and where it is not, one weak skill means re-taking the entire test. This single question changes your whole re-sit strategy, so confirm it with AHPRA / the Nursing and Midwifery Board of Australia directly rather than assuming.
• Your score expires, and registration is slow. Test scores are generally accepted for two years from the test date, while registration — documents, credential verification, regulator assessment, then a visa — routinely runs longer than people expect. Sitting the test as early as possible is a common instinct and a genuine risk: a score that expires mid-application means sitting it again. Time it against your realistic submission date, not against your ambition.
• The alternative test is worth a serious look. OET at grade B in each section is also accepted; results can be combined across two sittings under set conditions. OET is built around healthcare tasks — a referral letter, a patient consultation — so for a working clinician the content is familiar and the vocabulary is yours already. That does not make it easier in general, but it removes the mismatch between an academic test and a clinical career, which for many candidates is exactly what the extra half band was costing.

A realistic sequence.
